The address is 12425 Knoll Road Suite 110, Elm Grove, WI
My office is located at 124th and Watertown Plank in Elm Grove, right on the border of Elm Grove and Tosa. When you turn South on 124th you'll come to a building as the road turns right. Thats me! The building is called Iron Horse Crossing. You can park in one of the few spots out front or there is plenty of available parking on the side and in back of the building.
If stairs make things more difficult, please park in a spot out front for building access.

iPad or cell phone:
Go to your app store and download the free SimplePractice Telehealth app. Its a blue icon with a video camera image. When you click the link to our appointment it will open up our video room in the app.
If you didn't get a link, please email me and I'll send you a new one!

You can reschedule your appointment by:
-Logging into your client portal and selecting “cancel” when you click on your appointment time.
-You can also email or call to let me know that you won’t be able to make your appointment.
You can always reschedule another appointment online through your client portal.
